Montgomery began her 30-year career at MUSC Hospital in 1991 as director of Patient Access Services, and in 1998, she was named director of Patient Financial Services and controller. Working in collaboration with institutional leaders, Montgomery led the hospital's conversion to the Medical University Hospital Authority, a move that allowed the hospital to operate more efficiently as a public benefit corporation. MUHA officially launched in July 2000. Because of her experiences and successes with the hospital, Montgomery was appointed by then-president Raymond S. Greenberg, MD, PhD, as Executive Vice President for Finance and Operations while continuing in her position as Chief Financial Officer of the MUSC Hospital Center. In her role as Executive Vice President for Finance and Operations, she was directly responsible to the MUSC President for providing the leadership and vision necessary to ensure effective oversight of all fiscal and general operations of the University.

Montgomery served on the President's Cabinet and senior leadership team, providing counsel to the University's administrative, academic, and healthcare leadership teams and MUSC's Board of Trustees. She served on the Board of Directors for MUSC Physicians, the West Edge Foundation, and the Barrier Islands Free Medical Clinic. Montgomery is past president of the South Carolina Healthcare Financial Management Association and served on the Board of Directors for five years. She also previously served on the Board of Directors for the South Carolina Hospital Association, the South Carolina Education Authority, the Charleston Metro Chamber of Commerce, and Trident United Way. Montgomery received the YQCA Women of Influence Award in 2004 and the TUW Role Model of the Year Award in 2018. She maintained professional memberships in the American College of Healthcare Executives, the Healthcare Financial Management Association, and the National Association of College and University Business Officers.

In addition to overseeing the institution's finances, Montgomery championed mentorship and promoting women and minorities to leadership roles. She collaborated with interim chief equity officer Willette Burnham-Williams, PhD, in setting diversity goals for Finance and Administration and adopted communications tools and practices to connect diversity and inclusion efforts. Upon retiring from MUSC in June 2021, Montgomery established needs-based scholarships to support students in healthcare leadership in the College of Health Professions. The Lisa Montgomery Endowed Scholarship is the college's largest endowed scholarship.
